How do I make a medical appointment?

An appointment can be made online through this website, or patients can call our office at (772) 581-8900


What insurances do you accept?

We accept the following insurances: Florida Medicare, Aetna, Cigna, United Healthcare, Health First, Careplus, Galaxy Health Network, Fortified Provider Network, Freedom Health, Optimum, Railroad, WellMed and WellCare.

Please note the following insurances are “Pending”: Blue Cross Blue Shield, Medicaid, Humana, TriCare.


What type of Vascular surgical procedures do you offer?

We provide comprehensive treatment for lower extremity complaints including varicose veins, leg swelling, non-healing wounds, and painful legs.


What do I need to bring to my first appointment?

Please bring your driver’s license, insurance card, and a form of payment if a Co-pay applies to you.


Do I need a referral to make an appointment?

Some insurance policies may require that your primary care physician refer you. If you would like to learn whether you need a referral or not, you can call us or your primary care physician.

Do you provide tele-medicine service?

We do provide telemedicine services on a restricted case-by-case basis given the nature of our “hands-on” practice. If you feel the need to conduct a telemedicine visit with our providers, please contact the office to inquire.


What can I do to take care of myself after my visit?

The providers will furnish you with information after your visit to detail how you can take care of yourself on a continuous basis.
